The live transcript sometimes stops while you are still speaking, and nothing says whether it is still recording
OpenHighFailureClaude app (iPhone and iPad)
What we saw
Mid-dictation the live transcript can stop updating while the waveform keeps moving, so there is no way to know whether the rest is being captured. Sometimes pressing the square then shows the missing text after a "transcribing" step; sometimes it has been lost. The reporter puts it at roughly half the time it stalls, and has lost "quite a lot of voice memos" this way. The audio is never offered back for a retry.

Why it matters
The reader has to keep a mental check running while they speak, watching for words to appear, and stops trusting the tool whenever they do not. When it fails the recording cannot be recovered or resubmitted, so the loss is total.
The fix
Show the state explicitly: "Live transcript paused, audio still recording" when the live pass stalls, and a clear error if capture itself stops. Keep the audio on the device until the final transcript is in the box, and offer "Transcribe again" when it fails.
What good looks like: EX-002 Dictation that shows your words as you speak them.
The record
| Status | Open |
| Severity | High |
| Kind | Failure: works against the reader |
| Product | Claude app (iPhone and iPad), app |
| Where | Message box, voice input |
| Problems | Missing feedback, Data loss |
| Visibility | Intermittent: some readers, some of the time: depends on a gesture, a device or a state, and is hard to reproduce on demand |
| Recommendation | Show the state: always show what the system is doing (listening, working, paused, failed) and keep what was captured until it is safely delivered |
| Area | Input and dictation |
| Breaks | The reader can always tell what the system is doing |
