The transcript is cleaned up in a second pass without saying when, or what changed

OpenLowFailureClaude app (iPhone and iPad)

What we saw

After the live transcript, a second pass rewrites the text to tidy it up. The result is usually better, but nothing shows that it happened, when it happened, or which words were changed from what was actually said.

Why it matters

The reader can no longer tell their own words from the model's interpretation of them, so they cannot tell what to check. For an instruction to an agent, a quietly changed word can change the task.

The fix

Mark the moment the second pass runs, highlight words it changed, and offer "show what I said" or a one-tap revert to the raw transcript.
